Community Care Worker, Team 12 - Duty & Assessment Service
Fixed Term Maternity Cover, full time (37 hrs per week),
Scale 5/6, Salary £19,621 - £23,708
Team 12 are looking to recruit a Community Care Worker to the busy Adult's Duty and Assessment Service to cover Maternity Leave.
The work involves staffing the Duty Desk where you will deal with customer enquiries and take referrals in relation to vulnerable adults. In addition you will complete Community Care Assessments, commission Reablement Services to support vulnerable people in their homes and request equipment from Occupational Therapy. The role involves meeting with and speaking to customers and their families, liaising with colleagues from other agencies and services to ensure the safety and wellbeing of local people. You will contribute to team development and work with supportive colleagues to deliver a first class service to vulnerable adults in Rutland.
We are looking for someone who has experience of working in a social care setting; someone with an ability to engage with customers and their families as well as partner agencies and service providers. You will have good communication skills and an understanding of the needs of elderly and disabled adults. Knowledge of completing Community Care Assessments and Continuing Health Care would be an advantage. You will be required to produce written assessments and reports within timescales and to manage competing demands under pressure. You will need to have good organisational skills.
