REACH Co-ordinator – Maternity Cover

Rutland Enablement, Assessment, Crisis Response and Health (REACH) & Social Care Integration Team

1 Fixed term post to cover maternity leave, Full time (37hours per week) on a flexible rota including mornings and evenings, out of office hours, weekends on a rota basis and bank holidays

Scale 5, Salary £20,661 - £22,658 pro rata

The REACH team enables service users to achieve their optimal level of independence and helps to establish any on-going services required to support them to remain living in their own homes. This includes people coming out of hospital or struggling to manage in the community.

You will work under the direction of the REACH Managers and work closely with the Therapists to help deliver this key service. You will plan the programmes and deploy the Reablement Support Workers to ensure the delivery of an effective and responsive Reablement service, using our Care Management 2000 computer system. You will need to use your own initiative and respond quickly to cope with emergency situations to provide out of hours support and advice to safeguard and promote the welfare and independence of service users.

We are looking for someone who has a commitment to providing a high quality service, delivered in a person centred way. You will require excellent organisational and leadership skills and be able to communicate effectively with multi-disciplinary and multi-agency colleagues.

You will need to have experience of working with older and disabled people and knowledge of relevant health and social care legislation and a commitment to ensuring the service complies with CQC’s standards of care.

The successful applicant for this post will be subject to an Enhanced check by the Disclosure and Barring Service (DBS).
